UNI Football Camps Every camper (this includes coaches attending the 7-on-7 and Team Camps) is required to complete a "Release and Medical Authorization" form. Please print the form, complete it, and mail to: UNI Sports Camps-Football Registration is not complete and you will not be allowed to participate until we have this form. If you registered online, a form was included with the confirmation letter.
